Key Responsibilities:

The Licensed Practical Nurse (LPN) is responsible for delivering direct patient care to residents in our health and rehabilitation center, assisting them with daily living activities under the guidance of a registered nurse.


Responsibilities include, but are not limited to:
  • Implementing personalized treatment plans for each resident.
  • Supervising Certified Nursing Assistants (CNAs) and participating in their performance evaluations.
  • Managing the Falls Prevention and Pressure Ulcer Prevention Programs.
  • Providing essential nursing services.
  • Conducting thorough assessments of each resident's medical condition upon admission and throughout their treatment journey.
  • Performing additional duties as assigned.

Qualifications:
  • Must hold a current, unencumbered state license to practice as an LPN.
  • Experience in skilled nursing or rehabilitation settings is preferred.
