Caregiver Specialist

About the Role:
- Evaluate patients' needs and develop personalized care plans to ensure their physical, emotional, and spiritual well-being.
- Provide empathetic support to patients and their families, addressing their concerns and needs with compassion and understanding.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
- Prioritize effective communication to maintain open lines of dialogue with patients, families, and healthcare professionals.
Qualifications and Requirements:
- A current state license as a registered nurse (RN) or equivalent experience in a clinical care setting.
- At least 2 years of experience in nursing, with a preference for those with hospice care experience.
- Demonstrated ability to work independently and collaboratively within a team environment.
- Reliable transportation and the ability to lift at least 25 pounds.
