About the Role

We are seeking a skilled Occupational Therapist to join our team at Jukebox Health. As a key member of our team, you will be responsible for conducting home safety evaluations with clients and/or caregivers to identify ways to improve safety and independence in the home.

**Key Responsibilities:**

  • Conduct thorough home safety evaluations to identify potential hazards and provide personalized recommendations for improvement.
  • Develop and implement customized home modification plans to enhance safety and independence for clients.
  • Provide education and training to clients and/or caregivers on fall prevention strategies and techniques.
  • Establish strong relationships with clients and/or caregivers to understand their unique needs and preferences.

About You

We are looking for a highly skilled and compassionate Occupational Therapist who is passionate about helping older adults live safely and independently in their homes. The ideal candidate will have:

  • New York State OT registration and licensure.
  • Experience working with the geriatric population, preferably with 1-3 years of experience.
  • Strong communication and clinical reasoning skills.
  • Able to work independently as an independent contractor.
