Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Hospice Case Manager. The ideal candidate will have experience in hospice care and be able to provide high-quality patient care.

Key Responsibilities:

  • Perform assessments and procedures, which require substantial specialized knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
  • Visit in-home clients to evaluate the quality of care being provided and review clinical notes.
  • Complete nursing initial and ongoing assessments of all in-home clients within specified deadlines.
  • Participate in ongoing interdisciplinary assessment of the patient.
  • Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Obtain physicians plans of treatment and orders, and initiate their execution.
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Counsel and educate the patient/client and family in meeting healthcare-related needs.
  • Effectively communicate with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
  • Secure equipment and supplies and coordinate placement for use in the home.
  • Participate in Quality Assessment and Performance Improvement (QAPI) program.
  • Participate in in-services or training, as requested.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in California. License is active and in good standing.
  • 1 year of accumulated experience as a registered nurse within the last five (5) years.
  • One (1) year of hospice care experience preferred, but not required: willing to do admissions, continuous care, supervisory visits, PRN and routine visits.
  • Able to provide proof of valid driver's license if applicable.
  • Able to provide proof of valid auto liability insurance if assignment(s) include driving vehicle.
