Job Title: Licensed Practical Nurse - Triage Specialist

We are seeking a skilled Licensed Practical Nurse to join our Medical Operations team at Primary Health Solutions. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and triage services.

Key Responsibilities:
  • Assist in examination and treatment of patients under the direction of a Provider.
  • Triage phone calls and walk-ins as needed, providing education and support to patients and families.
  • Document all patient communication and interactions in patient charts.
  • Support patients in basic self-management goal setting and apply active listening and behavior modification principles.
  • Assist with routine office examinations and procedures, including administering injections, medications, and blood draws.
  • Document all procedures and patient interactions, tracking and attaching laboratory and diagnostic test results to patient charts.
  • Communicate results and orders to patients as directed by the Provider.
  • Order new and refill prescriptions and follow up with patients as needed.
Requirements:
  • Graduation from an accredited school of nursing.
  • LPN certification and current, valid license to practice practical nursing in the state of Ohio.
  • Ability to read and interpret documents, write routine reports, and speak effectively before groups.
  • Ability to solve practical problems and deal with a variety of concrete variables.
Work Environment:

The work environment is fast-paced and dynamic, with a moderate noise level. The employee is occasionally exposed to fumes or airborne particles, toxic or caustic chemicals, and risk of radiation.
