About the Role

As a Registered Nurse in our Home Health team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Our team is dedicated to delivering compassionate and personalized care, and we are seeking a skilled and compassionate RN to join our team.

Key Responsibilities
  • Provide direct patient care, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.
  • Participate in ongoing education and professional development to stay current with best practices and industry advancements.
Requirements
  • Current Oregon Registered Nurse License.
  • 1 year of experience in medical/surgical care, with a focus on home health.
  • National Provider BLS - American Heart Association certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
