Lutheran Services Florida (LSF) is dedicated to transforming the lives of children, families, and communities. We are seeking a compassionate and detail-oriented Case Manager to join our team.

Estimated Salary: $45,000 - $55,000 per year, depending on experience

About the Role

The Case Manager will be responsible for ensuring the well-being and safety of youth in our care. This includes conducting initial intake interviews, gathering information from family members and sponsors, and determining options for reunification. The ideal candidate will possess excellent communication skills, a strong work ethic, and the ability to work independently.

Key Responsibilities
  • Conduct thorough assessments of youth needs and develop personalized service plans
  • Maintain accurate and up-to-date records of client interactions and progress
  • Collaborate with internal teams and external stakeholders to ensure seamless support services
  • Develop and maintain relationships with community partners and resources
  • Provide regular updates to youth and their families, including face-to-face meetings and telephonic contacts
Requirements
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field
  • 1 year of experience working with adolescents or in the youth services field (volunteer and internship experience included)
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment and adapt to changing circumstances
