Job Summary

The Occupational Therapist will work with clients to build individual rehabilitation plans designed to help them develop or regain physical or mental functioning through occupational-based activities in the client's natural setting.

Key Responsibilities:

Collaborate with LSSI employees to ensure client-centered care for all class members;
Coordinate to schedule client screenings and assessments in the community;
Provide assessments for class members recommended by LSSI;
Assess and evaluate client care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment;
Complete narrative reports on assessment outcomes with recommendations and deliver any intervention and/or skill building as needed for the class member to transition to independent living;
Assess client's personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course;
Develop and implement individualized treatment plans based on client needs containing achievable functional goals;
Conduct interviews with family members, guardians, and/or significant others, as appropriate, about class member's past and present functional levels, capabilities, and performance;
Attend meetings to consult with teams regarding client progress, behavior, and response to treatment;
Collaborate with interdisciplinary professionals to provide comprehensive care to support the client's successful independent living;
Comply with evaluation, treatment, and documentation guidelines;
Perform environmental evaluations and suggest modifications to increase safety and independence in the home as needed;
Provide direct supervision to Level I and Level II students as assigned;
Perform other related duties and/or projects as assigned.

Requirements

Master's Degree in Occupational Therapy required;
Qualified Mental Health Professional as defined by the MRO;
Licensed Occupational Therapist in the State of Illinois required;
Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance;
Minimum one year experience working with people with mental health and/or substance abuse diagnoses;
Experience in community-based services highly preferred.
