Job Summary:

The Physical Therapist will be responsible for screening, evaluating, and treating patients/residents to optimize their mobility and activities of daily living. This role requires a graduate of an accredited Physical Therapy program with one year of experience in a geriatric rehabilitation setting. The ideal candidate will have a current license to practice as a Physical Therapist in the facility-specific state and certification in Basic Life Support. Reliable transportation is also required.

Key Responsibilities:

• Provide professional Physical Therapy services to patients/residents
• Complete documentation accurately and timely
• Participate in Quality Assurance and Performance Improvement initiatives
• Work efficiently and effectively while utilizing facility resources responsibly
• Maintain adequate and up-to-date knowledge of Physical Therapy, Rehabilitation Services, and setting-specific clinical and regulatory practices
• Comply with facility policies and procedures
• Supervise Physical Therapist Assistants, therapy aides, and therapy students as needed

Requirements:

• Graduate of an accredited Physical Therapy program
• One year of experience as a Physical Therapist (geriatric rehabilitation setting preferred)
• Current license to practice as a Physical Therapist in the facility-specific state
• Current certification in Basic Life Support
• Reliable transportation
