About the Role

BrightSpring Health Services is seeking a highly skilled and motivated pharmacist to join our team as a Long-Term Care Specialist. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to our residents in long-term care facilities.

Responsibilities

  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient/resident databases, including medical history, clinical data, and medication information.
  • Ensure timely and effective laboratory testing for high-risk conditions, collaborating with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement and maintain policies and procedures for safe medication management, procurement, storage, and disposal.
  • Inform the Pharmacy Manager of medication-related errors and assist with procedures related to safe medication practices.
  • Perform other tasks as assigned by the Pharmacy Manager.

Requirements

  • Bachelor's Degree in Pharmacy; PharmD preferred.
  • One year of experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
