At Sanzie Healthcare Services Inc, we are seeking a skilled and compassionate LPN to join our team as a Supervisor. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care and supervision to our clients in their homes.

• Establish and implement client service plans in coordination with our staff.
• Provide skilled nursing care and supervision to clients according to their individual needs.
• Conduct initial face-to-face assessments of clients at their homes.
• Regularly assess the nursing needs of clients and report any changes to their physicians.
• Supervise CNAs and other home health aides to ensure high-quality care.
• Perform supervisory visits to clients to observe caregivers and advise on proper care and procedure.
• Assist with orientation of new caregivers and annual evaluations.
• Provide education to clients and their families on proper care and treatment.
• Revise client plans in consultation with physicians based on ongoing assessments.

Requirements:

• Current LPN licensure.
• Experience in home healthcare or a related field.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of medications and their administration.
