Job Description
In this role, you will work as an independent practitioner, responsible for providing and directing provisions of nursing care to patients in their homes. You will collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and effective communication.
Your Responsibilities
- Initial and ongoing assessments of patients' physical, emotional, and social needs.
- Development of individualized care plans that address patients' goals, values, and preferences.
- Implementation of evidence-based practices to promote patient safety, quality, and satisfaction.
- Maintenance of accurate and complete records, including progress notes, medication lists, and treatment plans.
Qualifications and Skills
- A valid RN license in the state of practice.
- Current CPR certification.
- Strong clinical judgment and decision-making skills.
- Effective communication and interpersonal skills.
