We are seeking a skilled Pediatric Private Duty LPN to provide high-quality nursing care to patients in their homes. As a member of our team, you will work under the supervision of a Registered Nurse and be responsible for providing direct nursing care, recording clinical and progress notes, and coordinating other patient care activities.
Responsibilities- Provide high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
- Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conference, and communicating effectively with the patient, family, physician, and other clinicians.
- Ensure confidentiality of patient health information per agency policy.
- Current LPN licensure in the state of Tennessee.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
