Job Summary

We are seeking an experienced Wellness Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will be responsible for promoting the overall well-being of our residents by delivering personalized care and support.

Responsibilities:
  • Conduct regular wellness visits to assess the physical and emotional needs of our residents
  • Develop and implement individualized care plans to address specific health needs
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Maintain accurate records and documentation of resident care
  • Communicate effectively with residents, families, and caregivers to ensure informed decision-making
Requirements:
  • CURRENT RN OR LPN Licensure REQUIRED
  • Minimum 1 year experience in home health, assisted living, hospital, or long-term care environment
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care
  • Able to work weekends, evenings, and flexible hours
