Job Description:

As a Physical Therapist at Senior Care Partners P.A.C.E., you will play a vital role in assessing and improving the functional mobility of our participants. This position requires a comprehensive understanding of physical therapy principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Conduct thorough assessments of participants' functional needs and develop personalized plans of care.
  • Provide evidence-based physical therapy interventions to improve or maintain function.
  • Collaborate with the Interdisciplinary Team to ensure seamless care coordination.
  • Oversee wellness programming and provide education to participants and caregivers.
  • Supervise Physical Therapy Assistants and provide guidance and support as needed.
Duties and Responsibilities:
  • Conduct initial and periodic assessments to identify participants' functional needs and develop a participant-centered plan of care.
  • Assess the medical necessity for adaptive and/or durable medical equipment and make recommendations to the Interdisciplinary Team.
  • Initiate orders for equipment and perform quality checks to ensure it meets participants' needs.
  • Develop and implement participant-centered plans of therapy, including therapeutic exercise, modalities, and manual therapy.
  • Participate in participant-centered care planning and actively engage with the Interdisciplinary Team.
  • Consult with the Recreational Therapy department to provide physical wellness programming groups at the center.
  • Provide education and training to caregivers, family members, and friends as needed to support the plan of care.
  • Communicate participant changes to team members and contribute to the development of the participant-centered plan of care.
  • Provide exceptional customer service to participants, visitors, and internally between departments and staff members.
  • Collaborate with the Interdisciplinary Team to ensure seamless care coordination and promote a high level of customer service.
  • Document in the participant's medical record as required by policy.
  • Complete all assigned company education and file requirements on time.
  • Participate in Quality Improvement activities.
  • Keep the leader informed of problems or issues.
  • May provide student oversight.
  • Perform other duties as assigned.
