About the Job:

As a Home Health Aide, you will perform delegated nursing, homemaking, and other delegated duties for patients of all ages and families in their homes or other designated facilities. You may also perform PT/INR checks in the home as ordered by the physician and report results according to policy.

What You'll Do:

  • Report and document patient/client's needs and responses to the appropriate Case Manager and/or Clinical Manager.
  • Report patient/client's change in condition or situations immediately.
  • Adhere to patient/client Home Health Aide care plan as set up the primary nurse/therapist while maintaining a healthy, safe environment.
  • Reinforce the teaching of the primary nurse/therapist by reviewing personal care techniques with caregivers and patients.
  • Complete documentation in clinical records, time sheets and quality control logs.
  • Attend a minimum of 12 hours of in-services per year.
  • Provide care while promoting client/patient dignity and provide a calm, supportive and therapeutic environment for client/patients and their families.
Who You Are:

Education: High school diploma or equivalent preferred.
Experience: Six (6) months to one (1) year of experience in a related field desired.
Certification/Licensure: Current Nursing Assistant License in the State of New Hampshire required.
Schedule: Work schedule may include 8 to 12 hours per day with weekend, evening, night, and on-call coverage based on SolutionHealth needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the service area of SolutionHealth to meet patient/client needs.
Other: Driver's License required to travel throughout New Hampshire to provide home health care service and other job-related activities. Must maintain automobile insurance coverage at established SolutionHealth minimum at all times of employment.
