Job Summary:

The Occupational Therapist will be responsible for providing high-quality patient care, including evaluations, treatment planning, and implementation of interventions to promote functional independence and overall well-being. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that address the unique needs of each resident.

Key Responsibilities:
  • Evaluate residents' physical and cognitive abilities to identify areas for improvement and develop targeted treatment plans.
  • Implement evidence-based interventions to promote functional independence, mobility, and overall well-being.
  • Collaborate with the interdisciplinary team to develop and implement individualized treatment plans.
  • Document resident progress and outcomes in a timely and accurate manner.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
Requirements:
  • Graduate of an accredited Occupational Therapy program.
  • Current, active license as an Occupational Therapist in the state of practice.
  • Experience working in a long-term care setting preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Working Conditions:

This position requires the ability to work in a fast-paced environment and adapt to changing situations. You will be required to lift up to 100 pounds with assistance and stand for extended periods. A valid driver's license and reliable transportation are also required.
