We are seeking a dedicated RN Coordinator to join our team and contribute to our mission of providing top-tier transplant care.
Position Summary and Responsibilities
The ideal candidate will be a Registered Nurse responsible for the assessment, planning, implementation, and evaluation of nursing care for patients with chronic organ diseases and those undergoing transplantation. The RN will coordinate care among the transplant team to ensure optimal patient outcomes.
Key Responsibilities:
Pre-Transplant Phase:
- Conduct necessary evaluations to determine patient eligibility for transplantation.
- Educate patients and families about the transplant process.
- Participate in patient selection and evaluation.
- Monitor patient progress throughout the work-up and listing phases.
- Prepare patients for transplantation surgery.
- Ensure compliance with regulatory data management requirements.
Transplant/Inpatient Phase:
- Lead educational initiatives for patients and families.
- Engage in clinical rounds to monitor patient quality and safety.
- Maintain communication with the multidisciplinary team.
- Serve as a resource for staff regarding transplantation.
- Adjust care plans based on ongoing assessments.
- Facilitate discharge planning and outpatient follow-up.
- Ensure continuity of care through effective communication with referring physicians.
Post-Transplant Phase:
- Monitor diagnostic studies and evaluate patient health status.
- Address patient concerns and coordinate with the transplant team.
- Continue patient and family education post-transplant.
- Provide care under the guidance of the transplant physician.
- Maintain regulatory data management compliance.
Qualifications:
- Graduate of an accredited nursing program (diploma, associate, or bachelor’s degree).
- Minimum of one year of RN experience, meeting all RN II requirements.
- Basic Cardiac Life Support certification required within 30 days of hire.
