Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments for older adults and other at-risk individuals in their homes, identifying potential health risks and providing personalized care recommendations.

Key Responsibilities
  • Conduct in-home health assessments, reviewing medical history, medication adherence, and social environment
  • Develop and implement personalized care plans to address identified health risks
  • Collaborate with healthcare providers, case managers, and social workers to ensure seamless care coordination
  • Provide education and support to patients and their families on healthy aging and disease prevention
  • Document all assessments and care plans in accordance with Matrix Medical Network guidelines
Requirements
  • Current licensure as a Nurse Practitioner in the state of Texas
  • Minimum 2 years of experience in geriatric care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
