Horizon Health is a Critical Access, Rural Health Facility with 25 inpatient beds located in Illinois. We offer a range of outpatient services, including family practice and specialty clinics, as well as emergency care.

Job Summary

We are seeking an experienced RN to join our team as a Care Coordinator. This role will involve working closely with interdisciplinary staff to provide patient-centered care, advocating for patients and their families, and coordinating healthcare services to ensure seamless transitions between care settings.

Key Responsibilities
  • Educate patients and their caregivers about chronic illness management and strategies to prevent hospital readmissions.
  • Collaborate with healthcare teams to identify and engage patients in care coordination activities.
  • Develop and implement individualized care plans with patients and their families, ensuring continuity of care and reducing fragmentation of services.
  • Act as a patient advocate, protecting confidentiality and upholding the highest standards of care.
  • Coordinate education, monitoring of health needs, and community resources to promote patient empowerment and quality of life.

This role requires strong communication and problem-solving skills, as well as the ability to work independently and collaboratively within a team environment.

Requirements
  • Current State of Illinois RN license required; BSN preferred.
  • ACLS and PALS certification necessary.
  • Minimum five years of acute care experience; home health experience desirable.
  • Demonstrated skills in communication, conflict resolution, and critical thinking.
Compensation and Benefits

Hourly rate: $30-$45 per hour (dependent on experience). Comprehensive benefits package includes medical, dental, and vision insurance, Employee 403(b), health savings account with Company match, and paid time off.
