This position offers the opportunity to work with patients and families in their homes, providing high-quality care and support during a challenging time.
Job Summary:
The successful candidate will provide nursing services requiring substantial and specialized skill, in accordance with individual patient plans of care. This includes assessing patient needs, developing and implementing personalized care plans, and coordinating with other healthcare professionals to ensure seamless care delivery.
Responsibilities:*
- Provide direct patient care, including administering medications, wound care, and pain management.
- Assess patient needs, develop, and implement individualized care plans.
- Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
- Educate patients and families on disease management, treatment options, and coping strategies.
- Document patient progress, noting changes in condition or response to care.
Requirements:
To be successful in this role, you will need:
- A current RN license in the state of practice.
- One year of experience in an acute-care setting, home health, community health, or hospice environment.
- Valid driver's license and reliable transportation.
- Proof of automobile insurance (MVR checked).
- Current CPR certification.
- Able to pass criminal background check and national sex offender clearance.
- Must have proof of up-to-date COVID vaccine or submit an exemption request.
