Job Summary

As a Nurse - Home Care/Hospice at LifeBridge Health, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing compassionate and personalized care to individuals and their families, and we are seeking a skilled and empathetic nurse to join our team.

Responsibilities
  • Assess patients' and families' needs and develop individualized care plans to meet their unique needs.
  • Provide professional, comprehensive, and family-oriented nursing care to patients in their homes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Perform primary care case management functions, including assessing patients' needs, developing care plans, and evaluating patient outcomes.
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • Minimum of two years of recent medical/surgical nursing experience, with one year of home health or hospice experience preferred
  • Expertise in performing health assessments and developing care plans
  • Certified Hospice Palliative Nurse (CHPN) certification within four years of hire or after meeting CHPN exam requirements
  • CPR certification from the American Heart Association (AHA)
