Hepatology Navigator and Care Coordinator

We are seeking a skilled Nurse Navigator Hepatology to join our team at UCHealth Anschutz Outpatient Pavilion in Aurora, CO.
Job SummaryThis is a full-time opportunity for a dedicated nurse to serve as a patient advocate, liaison, and advisor/educator. As a Nurse Navigator Hepatology, you will assist patients with navigating the continuum of care and provide emotional support, clinical education, and expert guidance.
Key Responsibilities:- Round on/review assigned patients regularly and evaluate their progress with a plan of care. Communicate this plan to the patient and family and solicit concerns, questions, and issues for resolution.
- Provide emotional support, counseling, clinical education, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
- Serve as a liaison between patients and clinical staff, administration, physicians, managed care companies, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.
- Assist patients and families with resolving financial, psycho-social, functional, and administrative issues by advising them of options and referring to appropriate resources. Intervene as necessary to advocate for patients and families.
- A graduate degree from an accredited or state board of nursing-approved Registered/Professional Nursing program (if less than 3 years of experience).
- A valid state license as a Registered Nurse (RN). Relevant life support certification determined at position level.
- At least 2 years of nursing experience.
- BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer, with a valid card in hand before starting work. BLS or CPR cards must be valid for at least 60 days after hire.

CompensationThe estimated annual salary for this role is $79,500-$123,900, based on the national average salary range for a registered nurse in Colorado with similar experience. Pay is dependent on relevant experience and qualifications.
