About the Role

We are seeking a skilled LPN LVN to join our team at Providence Facey Medical Foundation. As a Licensed Practical Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our healthcare team.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Graduate of an accredited licensed practical/vocational nursing program.
  • Current California Licensed Vocational Nurse (LVN) license.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • At least 18 months of licensed nursing experience.
Preferred Qualifications
  • One year of office nursing experience.
