We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team at AssistCare, a licensed and Medicare-certified home health agency providing skilled healthcare services in the Northeast Texas area.

Job Summary:

The successful candidate will provide quality, comprehensive nursing care in the patient's home under the direction of a Registered Nurse (RN). Responsibilities include collaborating with the interdisciplinary team to develop the plan of care for the patient, assisting the patient in learning all appropriate self-care techniques, and reporting to the RN all client problems and assisting with modifying care plans accordingly.

Requirements:
  • Skilled visits
  • IV Certification
  • Current Nursing License in the state of Texas
  • Minimum of one year of experience as a licensed vocational nurse
  • Previous experience in Geriatric Home Health
  • Kinnser/Wellsky experience is a plus
  • Ability to speak, read, and write English
  • Must have and maintain a valid Texas driver's license and proof of current auto liability insurance
  • Must maintain current certificate of basic life support (CPR)
