About the Role
This position provides direct patient care, including assessment, planning, implementation, and evaluation of patient outcomes. You will work closely with our interdisciplinary team to ensure seamless delivery of care and provide ongoing education to patients and families. The ideal candidate will have experience in chronic/acute hemodialysis and possess excellent communication and leadership skills.
Responsibilities
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient outcomes
- Collaborate with the interdisciplinary team to ensure seamless delivery of care
- Provide ongoing education to patients and families
- Develop and implement individualized plans of care based on patient needs
- Monitor and document patient progress, making adjustments as needed

Requirements
- Current state licensure as a Registered Nurse (RN)
- Bachelor's degree in nursing required
- Experience in chronic/acute hemodialysis preferred

- Physical stamina to lift up to 200 pounds and stand for prolonged periods
