Role Summary: Under the guidance of the Director of Nursing, the MDS Coordinator/RN plays a pivotal role in managing the MDS/Resident Assessment Instrument. This position is crucial for ensuring compliance with state regulations and for the ongoing education of team members involved in the MDS process. The coordinator is responsible for the electronic submission of MDS data to the relevant health authorities and must demonstrate proficiency in completing the MDS/Resident Assessment Instrument. Effective communication within the interdisciplinary team is essential.

Key Responsibilities:

  • Conduct and finalize MDS and RAP assessments in accordance with state and federal regulations.
  • Input necessary data into the electronic health record system.
  • Provide training and support to interdisciplinary team members regarding their contributions to the MDS process.
  • Perform resident interviews as required for MDS completion.
  • Ensure timely transmission of MDS data to the State Department of Health.
  • Engage in Aims testing and participate in medical rounds, orders, and consultations.
  • Contribute to the Fall and Psychosocial committees.
  • Participate actively in interdisciplinary care conferences.
  • Develop initial nursing care plans and oversee their execution.
  • Screen potential admissions for the Nursing Facility.
  • Utilize the nursing process to create and implement comprehensive care plans that optimize patient outcomes.
  • Address the educational needs of patients, families, and colleagues using effective teaching strategies.
  • Delegate nursing responsibilities based on staff qualifications and patient care requirements.

Qualifications:

Education: A nursing degree from an accredited nursing program.

Licensure: Must hold a valid registered nursing license in the state of ND or a compact state. Graduate Nurses (G.N.) must work under the supervision of a registered nurse while their practice authorization is active.

Experience: Basic training in MDS is preferred.

Compensation:

Competitive pay rate of $36.92 per hour.
