Job Summary

We are seeking a highly organized and detail-oriented Medical Records Coordinator to join our team at Trellis Centennial. As a key member of our healthcare team, you will be responsible for maintaining accurate and up-to-date medical records, ensuring compliance with regulatory requirements, and providing exceptional customer service to our residents and families.

Key Responsibilities
  • Accurately record and maintain medical and administrative information in accordance with established charting and documentation policies and procedures.
  • Prepare and submit reports, such as the Daily Census Report, to the Nurse Supervisor/Charge Nurse in a timely manner.
  • Ensure that all personnel involved in providing care to residents are aware of their care plans and refer to them prior to administering daily care.
  • Identify and report discrepancies found in transcribing physicians' orders, diet orders/changes, charting, etc.
  • Review medication cards for completeness of information, accuracy in transcription, and legibility.
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided and the resident's response to care.
  • Notify the resident's next of kin of changes in their condition as instructed.
  • Assist in admitting, transferring, and discharging residents, as well as arranging for transportation and packing their belongings.
  • Complete necessary medical and administrative records upon admission, transfer, and/or discharge.
  • Attend committee meetings and participate in in-service training programs as required.
Requirements
  • High school diploma or GED required.
  • Exceptional communication and customer service skills.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
Work Environment

The work environment is typically low to moderate noise level. The employee must occasionally lift and/or move up to 25 pounds and may be required to assist in the evacuation of residents during emergency situations.
