Job Summary

We are seeking a compassionate and skilled Licensed Practical/Vocational Nurse (LPN/LVN) to join our interdisciplinary team at HUMANITY HOSPICE, LLC. As a LPN/LVN, you will play a vital role in providing direct patient care and working collaboratively with our team to achieve positive patient outcomes.

Key Responsibilities
  • Provide Direct Patient Care: Implement current nursing practice following a comprehensive assessment and the plan of care, providing treatments and working collaboratively with the team to help meet positive patient care outcomes.
  • Assess and Educate Patients: Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Coordinate Hospice Services: Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
  • Manage Pain and Symptoms: Provide appropriate pain/symptom management, evaluating patients' response to treatments/medications.
  • Participate in On-Call Duties: Participate in on-call duties as defined by the on-call policy.
  • Continuing Education: Participate in personal, professional growth and development, also participating in organization's quality assessment performance improvement teams and activities.
Requirements
  • Education: Graduate of an accredited practical nurse or vocational nursing program.
  • Experience: Three years nursing experience, community health/hospice or medical/surgical experience is preferred.
  • Licensure: Currently licensed as an LPN/LVN in the State.
  • Skills: Demonstrates good verbal and written communication, and organization skills.
  • Transportation: Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • CPR Certification: Possesses and maintains current CPR certification.
