Job Summary

We are seeking a compassionate and skilled LPN to join our team. As an LPN, you will provide nursing care in accordance with the client's plan of care, including comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion, and prevention coordination of services.

Responsibilities
  • Provide nursing care to residents in accordance with their individual plans of care
  • Conduct comprehensive health and psychosocial evaluations to identify residents' needs
  • Monitor residents' conditions and report any changes or concerns to the healthcare team
  • Develop and implement health promotion and prevention programs to improve residents' overall well-being
  • Coordinate services with other healthcare professionals to ensure seamless care delivery
Requirements
  • Two years of LPN experience required
  • Two years of experience in assisted living or a nursing home
  • Licensed Practical Nurse license in good standing
  • Ability to ensure community follows all federal, state, and local laws and regulations as pertains to resident services and care
  • Good judgment, problem-solving, and decision-making skills
  • Effective organization, time management, and written and verbal communication skills
  • Ability to handle multiple priorities and delegate assignments
  • Dedication to seniors and their well-being
  • Ability to work nights and weekends
