As a Home Health Aide, you will work directly with clients in their homes to provide robust, around-the-clock care, assisting with daily living activities, self-care tasks, and more.
Responsibilities- Perform home visits for patients as delegated by the care managing RN
- Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
- Support the patient and family as appropriate to the patient's condition
- Current certification as a HHA in Connecticut
- Valid drivers license
- Reliable transportation and willingness to travel in Windham County
- Strong communication and interpersonal skills
- Home health experience preferred
- Flexible schedule
- Mileage paid between patient visits
- Training, professional development, and technology
