As a Home Health Aide, you will work directly with clients in their homes to provide robust, around-the-clock care, assisting with daily living activities, self-care tasks, and more.

Responsibilities
  • Perform home visits for patients as delegated by the care managing RN
  • Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
  • Support the patient and family as appropriate to the patient's condition
Requirements
  • Current certification as a HHA in Connecticut
  • Valid drivers license
  • Reliable transportation and willingness to travel in Windham County
  • Strong communication and interpersonal skills
  • Home health experience preferred
