Job Summary

We are seeking a highly skilled and compassionate Full Time Nurse - Resource Coordinator to join our team at BayMark Health Services. As a key member of our staff, you will play a vital role in assisting individuals struggling with Opioid Use Disorder (OUD) in receiving medication-assisted treatment and counseling services.

Key Responsibilities
  • Client Support: Meet with clients to gather information for the Government Performance and Results Act, track client progress, and log/report data as required.
  • Program Management: Participate in program recruitment and referrals, monitor and report program data, and oversee the LaSOR Care Coordination process among participating agencies.
  • Resource Coordination: Ensure clients receive comprehensive support, including referrals for benefits, transportation, and other essential services.
  • Communication: Develop and maintain strong relationships with clients, vendors, and stakeholders to ensure seamless service delivery.
  • Administrative Tasks: Verify insurance status, assess eligibility for Medicaid, and complete NARCAN training with clients.
Requirements
  • Education: LPN/RN or Bachelor's degree preferred.
  • Experience: Minimum of two years of professional experience, with project management and community/public relations experience strongly preferred.
  • Credentials: Counselor credentials/license per Louisiana state requirements preferred.
  • Skills: Exceptional oral and written communication skills, proficiency in Microsoft Office, and knowledge of evidence-based treatment options and treatment systems.
