Responsibilities
  • Provide skilled nursing care to patients in their homes, as ordered by physicians and outlined in individual care plans.
  • Collaborate with supervising RNs to review caseloads and ensure timely communication of patient needs.
  • Evaluate patient assessment findings and communicate relevant information to healthcare team members and physicians.
  • Guarantee that changes in patient clinical status are communicated to relevant parties, as per Agency policy.
  • Educate patients, caregivers, and other staff members as needed.
  • Participate in case conferences to discuss multidisciplinary team responsibilities and patient progress.
  • Recommend referrals to other disciplines and community resources as necessary.
  • Follow medication rights and reduce the potential for medication errors.
  • Follow up on lab results and notify physicians of significant changes.
  • Participate in the agency's Quality Assurance Performance Improvement (QAPI) program.
  • Identify and mitigate risks to patient safety and well-being.
  • Complete accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Experience as an LPN/LVN, preferably in an acute care setting within the last 24 months or six months in home care within the last 12 months.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
