We are seeking a dedicated Registered Nurse (RN) to join our team at Cottonwood Healthcare and Rehabilitation Center. As a Senior Living MDS Coordinator - Care Planning Specialist, you will be responsible for coordinating the facility's Medicare, Resident Assessment Instrument (MDS), and care planning process. This role involves assessing and evaluating the quality of care for long-term care residents, developing care plans, conducting meetings with multidisciplinary teams, and more.

Key Responsibilities:

  • Assess and evaluate the quality of care for long-term care residents
  • Develop and implement care plans to meet the needs of residents
  • Conduct meetings with multidisciplinary teams to discuss resident care
  • Coordinate the facility's Medicare and MDS processes

Qualifications:

  • Graduate of an accredited nursing program
  • Valid, unrestricted RN license
  • Previous MDS experience preferred
  • Previous long-term care experience preferred, but not required
