We are seeking a dedicated Registered Nurse Case Manager specializing in Hospice care. This role is available on a PRN/Part-Time basis.

The Registered Nurse Case Manager is responsible for overseeing the implementation of the Plan of Care for each hospice patient. This role involves ongoing evaluation of both personal and professional performance, making necessary adjustments to enhance productivity and the quality of care provided.

  • Conduct initial nursing assessments to determine eligibility for hospice services within forty-eight (48) hours of referral.
  • Assess the physical, psychosocial, and environmental needs of patients and families, re-evaluating at least every fifteen (15) days.
  • Initiate and coordinate the comprehensive plan of care.
  • Document identified issues, relevant goals, interventions, and patient/family responses to hospice services.
  • Collaborate daily with patients, families, attending physicians, and other interdisciplinary team members to provide holistic care.
  • Educate and guide patients and families in self-care techniques when appropriate.
  • Maintain accurate and pertinent clinical documentation regarding the patient's condition.

Experience Requirements

  • A minimum of one year of clinical experience is required.

License Requirements

  • Current RN licensure in the state of practice along with one year of clinical experience.
  • Current CPR Certification.
  • A valid driver's license, vehicle insurance, and access to reliable transportation, or public transit.
