As a Licensed Practical Nurse, your responsibilities will include:
- Providing care to patients with illnesses, injuries, and chronic diseases, often after hospital discharge under the supervision of an RN
- Working with a team of physicians, RNs, CNAs, and aides to deliver quality patient care
- Taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor
- Assisting the RN in carrying out patient care plans and teaching patients self-care techniques
- Educating families on patient care plans, medication, and home safety
- Collaborating with the interdisciplinary team to coordinate patient care and plan for discharge
To be successful as a Home Health Licensed Practical Nurse, you'll need:
- Graduation from an accredited practical nursing program and an active LPN license in New Jersey
- Minimum of 1200 hours of LPN experience, or completion of the office's preceptor program
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills, and compassion
