Job Title: Transition Counselor

Join our team at Education Success Network as a Transition Counselor, where you will play a vital role in assisting students and their families with academic, career, college, and vocational planning.

Job Summary:

The Transition Counselor will work closely with students, families, and school staff to develop and implement transition plans that cater to individual needs and goals. This role requires a strong understanding of special education, career development, and post-secondary planning.

Key Responsibilities:
  • Counsel students in the special education setting, focusing on academic, career, and college goals.
  • Develop and implement transition curriculum, including career interest inventories, career awareness resources, job skill development, and transferable skills.
  • Conduct age-appropriate transition assessments and annual guidance reviews with students aged 12+, highlighting strengths, challenges, interests, life skills, learning style, career goals, and post-secondary plans.
  • Collect and maintain college/vocational/career resource materials for student use, assisting with college searches, application completion, financial aid/scholarships, and students with disabilities information.
  • Act as a liaison with ACCES VR, school district counselors/transition coordinators, and BOCES, attending counseling and transition-related meetings monthly.
  • Ensure secondary students accumulate and earn necessary credits and coursework towards graduation.
  • Consult and collaborate with staff regarding students' academic, social-emotional needs, strategies, and management plans.
  • Review 12th graders' IEPs and create an Exit Summary for each student.
  • Attend CSE meetings for students in grades 9-12.
  • Develop a master schedule, in collaboration with administration, and maintain class schedules for all students, ensuring compliance with graduation requirements.
  • Plan and host transition-related events for students to learn about different careers through panel discussions and guest speakers.
  • Plan and host scholarship/financial aid information nights for parents of high school students.
  • Develop and issue a transition newsletter for parents and students.
  • Coordinate and chaperone students on visits to BOCES, colleges, career-related field trips, and ACCES VR.
  • Provide SAT/ACT information, registration, and administration, ensuring test accommodations are provided and acting as a proctor for special tests.
  • Maintain professional competence and skills through in-service education, consultation with administration and colleagues, independent study, membership in professional organizations, and self-selected professional growth activities.
  • Participate in school-sponsored activities and events.
Knowledge, Skills, and Abilities:
  • Ability to maintain professional confidentiality concerning individual student data and achievement.
  • Maintain a high level of professional integrity.
  • Knowledge of NYS graduation pathways and requirements.
  • Demonstrate good interpersonal communication and problem-solving skills needed to handle behavioral situations and work effectively with colleagues/parents/guardians on student issues.
  • Organizational, time management, and planning skills.
  • Knowledge of special education, including accommodations in educational settings.
  • Demonstrate excellent written communication skills.
  • Ability to be flexible in interpreting and addressing student needs.
Minimum Qualifications:
  • NYS Certification as School Counselor. Nationally certified school counselor preferred.
Preferred Qualifications:
  • Five years prior counseling experience at high school level preferred but not required. Experience should preferably include working with individuals with a variety of learning profiles and needs.
  • Work-based learning coordinator extension preferred, but not required.
Scheduling:
  • Able to work flexible hours to accommodate parent/guardian meetings, school activities, and other mandated events/meetings.
  • Teachers must report by 7:30am on school days.
Salary:
  • $48,253-$61,197/year based on experience.
