Key Responsibilities- Care Coordination: Assess the health care needs of our members and develop a personalized care plan to ensure they receive the necessary services and support.
- Service Planning: Collaborate with primary care physicians and other healthcare professionals to create a comprehensive service plan that meets the unique needs of each member.
- Resource Identification and Referral: Identify and connect members with community resources, services, and support groups to help them navigate the healthcare system.
- Documentation and Communication: Maintain accurate and up-to-date documentation of member interactions and care plans, and communicate effectively with healthcare teams and community agencies.
- Education: Master's degree in social work or a minimum of six years of experience in a directly related field.
- License: Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Experience: One year of social work experience, with a focus on care coordination and case management.
