Job Summary

We are seeking a highly skilled and experienced Registered Nurse to serve as our MDS Coordinator in our long term care facility. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively within an interdisciplinary team.

Key Responsibilities

  • Ensure accurate and timely completion of all resident assessment instruments, including MDS sheets, face sheets, trigger sheets, RAP summaries, and care plans.
  • Teach and in-service new staff members on the process and laws governing resident assessment, ensuring they understand their roles and responsibilities.
  • Update staff on new and/or revised laws and interpretations of laws governing resident assessment, maintaining compliance with regulatory requirements.
  • Upon admission, log patients in the appropriate calendar and assist in initiating the assessment process, ensuring seamless transitions.
  • Assure the Minimum Data Set (MDS) is accurately completed and signed by each discipline before the end of each patient's assessment schedule, maintaining high-quality care.
  • Coordinate with interdisciplinary teams to develop comprehensive care plans, addressing residents' unique needs and preferences.
  • Sign off on MDS completion, ensuring accuracy and attention to detail.
  • Complete nursing sections of the MDS or assist as needed, utilizing expertise to ensure high-quality care.
  • During the 14-day assessment period, identify triggers and pull corresponding RAPs for completion, streamlining the process.
  • Conduct informal team meetings to discuss care planning for residents, fostering collaboration and open communication.
  • Oversee care planning from day 14 through completion on day 21, providing guidance and support as needed.
  • Maintain records of care planning and progress, tracking outstanding needs and addressing them promptly.
  • Report on continuing quality assurance and improvement activities to the CQI committee, contributing to facility-wide initiatives.
  • Electronically transmit MDS assessments per schedule, but at least monthly, ensuring timely submission.
  • Track submissions, discharges, re-entries, and status changes using logs, maintaining accurate records.
  • Prepare reports as required, providing insights into care planning and quality assurance.

Requirements

  • Registered Nurse with current Massachusetts License.
  • Two years of previous experience in long-term care.
  • Knowledge of DPH, OBRA, JCAHO, Medicare Regulations, and MDS process.
  • Experience working within an interdisciplinary team approach to care.
  • Computer skills and experience with electronic medical charts required.
  • Knowledge of growth and development across the lifespan.

Estimated Salary Range

$45.00 - $50.00 per hour, based on experience and qualifications.
