Job Summary

Centria Healthcare is seeking a dedicated and compassionate Private Duty Home Health Nurse - Licensed Practical Nurse to provide exceptional care to our clients across the state of Michigan.

Key Responsibilities
  • Develop and implement patient care plans in collaboration with the clinical team
  • Provide skilled nursing care to patients with complex medical needs, including TBI, SCI, pediatrics, and geriatrics
  • Administer medications, perform wound care, and provide other nursing interventions as needed
  • Conduct thorough assessments of patients' physical, mental, and emotional conditions
  • Document all patient interactions and care provided in a timely and accurate manner
  • Communicate effectively with the clinical team to ensure optimal patient outcomes
Requirements
  • Current state license as an LPN
  • Valid driver's license and reliable transportation
  • Current negative TB skin test or negative chest X-ray
  • Current flu vaccine
  • CPR certification
  • Excellent oral and written communication skills
  • Ability to follow verbal and written instructions
  • Ability to utilize computers/tablets and computer software
