Key Responsibilities
  • Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Provide comprehensive nursing care, including physical assessments, medication management, and wound care.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct regular patient assessments to identify changing needs and adjust care plans accordingly.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements
  • Current RN licensure in the state of Iowa.
  • One year of experience in hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic knowledge of electronic health records and Microsoft Office products.
Preferred Qualifications
  • BSN or MSN degree.
  • Experience with documentation in electronic health records.
  • CHPN certification.
