Job Description

As a Home Health Physical Therapist, you will work closely with patients and their families to support their health journeys. You will deliver compassionate and individualized care in home health settings, leveraging your expertise to improve patient outcomes and quality of life.

Key Responsibilities:
  • Provide physical therapy services to patients in their homes, focusing on improving mobility and overall health
  • Develop and implement personalized treatment plans, taking into account each patient's unique needs and goals
  • Collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's degree in physical therapy from a CAPTE-accredited program
  • Current licensure to practice as a physical therapist within the state of agency operation
  • Valid CPR certification
  • A minimum of nine months' experience as a physical therapist, preferably in community or home health settings
Estimated Salary Range

The estimated salary range for this position is $100,000 - $115,000 per year, depending on years of experience.
