We are seeking a skilled Social Worker to join our team at West Virginia University Hospitals. As a Social Worker, you will play a vital role in providing psychosocial support to patients and their families, helping them navigate the complexities of cancer care.
Key Responsibilities- Conduct comprehensive psychosocial assessments to identify emotional, social, and community strengths and problems related to patients' diagnoses, illnesses, treatments, and life situations.
- Develop and implement individualized treatment plans, utilizing evidence-based interventions and therapies to address patients' needs.
- Provide education and support to patients and families on adapting to diagnoses, illnesses, treatments, and life situations.
- Collaborate with interdisciplinary treatment teams to ensure comprehensive care and coordinate discharge planning.
- Facilitate referrals to community resources and placement options, including personal care homes, nursing homes, skilled rehab, assisted living, and group homes.
- Document assessments, plans, interactions, and interventions according to departmental, hospital, and health system guidelines and standards.
- Master's Degree in Social Work or related field.
- Current licensure as a Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through the West Virginia Board of Social Work.
- Two (2) years' social work experience, preferably in oncology.
- Certification in Case Management.
- Previous oncology experience.
The Social Worker will work in a fast-paced, dynamic environment, collaborating with healthcare professionals to provide high-quality patient care. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team setting.
