Job Summary

The Community Health Worker is a vital member of the University Hospital Newark team, dedicated to improving the health and well-being of our patients. As a trusted liaison between the hospital and the community, you will develop strong relationships with patients and families, connecting them to essential resources and services.

Responsibilities
  • Develop and maintain relationships with patients and families to understand their unique needs and challenges.
  • Identify and connect patients to community resources, including physical and mental health services, to address social barriers to care.
  • Collaborate with clinical teams to enhance care management strategies and provide patients with access to preventive care screenings and services.
  • Participate in community outreach and engagement efforts to promote health equity and address health disparities.
  • Provide education and support to patients and families on healthy behaviors, disease management, and wellness strategies.
Qualifications
  • High School Diploma or equivalent required; Associate's Degree preferred.
  • Two (2) years of recent experience working in a community-based setting, with a focus on health and human services.
  • Strong interpersonal skills, with the ability to work effectively with diverse populations.
  • Basic computer skills and ability to navigate electronic health records.
  • Ability to work independently and as part of a team.
Preferred Qualifications
  • Bilingual or multilingual skills, with proficiency in Spanish, Portuguese, or Haitian Creole.
  • Experience working in a substance abuse environment or with individuals affected by interpersonal violence.
  • Completion of a Community Health Worker training program.
