Key Responsibilities- Collaborate with our clinical teams to provide advanced illness management and palliative care services to patients in their homes or in our facilities.
- Conduct comprehensive assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Prescribe medications, order laboratory and imaging tests, and provide other treatments as needed to manage patients' symptoms and improve their quality of life.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Master's degree in Nursing from an accredited program.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
- Minimum of two years of experience as a Nurse Practitioner in a palliative care or hospice setting.
- Strong leadership and interpersonal skills, with the ability to work effectively in a team environment.
- Excellent communication and documentation skills, with the ability to maintain accurate and timely records.
