In collaboration with patients, physicians, and the interdisciplinary team, the Case Manager facilitates efficient and cost-effective patient care progression through the acute episode.

Monitors care levels, initiating evidence-based protocols and order sets according to admission diagnoses, influencing care progression and proposing transition plans within 48 hours of admission.

The Case Manager serves as a patient and family advocate throughout the acute episode of care.

Education Requirements:

Bachelor's Degree: in Nursing (BSN) or a combination of education and experience in lieu of a BSN with a commitment to obtain within 2 years from hire date.

Experience: 1 year of experience in acute care Case Management or Utilization Management at an acute care facility and/or insurance company in either UR or CM.

General Experience: knowledge of nursing theories, principles, and concepts, as well as strong leadership capabilities.

Preferred Experience: knowledge of InterQual criteria.

License/Certifications: RN-LIC - PA Registered Nurse License and CCM - Certified Case Manager.
