Overview:

Jewish Family and Children's Services (JFCS) is committed to providing exceptional support to children and families in need. As a High Needs Case Manager, you will play a vital role in helping children achieve a more self-sufficient lifestyle and/or family reunification.

Responsibilities:

  • Provide High Needs Case Management services to children enrolled in the behavioral-health system.
  • Establish and maintain regular contact with assigned clients, including home/community visits and attending medication checks.
  • Assist youth in navigating discharge from hospitals, finding intensive services as clinically-indicated, and working to decrease behaviors to improve stability in the home and community.
  • Coordinate services and collaborate with other involved agencies, such as the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Coordinate and facilitate Child & Family Team (CFT) Meetings.
  • Complete referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
  • Collaborate with the clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork are completed accurately and in a timely manner.

Qualifications:

  • Meet one of the following education/experience requirements:
  • High School Diploma/GED required and 4 years of Behavioral Health experience.
  • A.A. Degree and two years of Behavioral Health experience.
  • Bachelor's Degree in Social Work, Psychology, or related field.
