Physical Therapist

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping our patients achieve optimal physical function and independence.
Responsibilities- Evaluate patient functional status and physical therapy needs, assisting the physician in developing a therapy plan of care.
- Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of MA.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
