Compassionate Senior Medical Care Provider

Job DescriptionThe House Calls Nurse Practitioner provides evidence-based medical care to seniors and patients with complex needs in their homes or assisted living facilities. This role involves evaluating medical needs, treating chronic and urgent conditions, and collaborating with other healthcare professionals to ensure comprehensive care.
The ideal candidate will have a Master's degree in Nursing (MSN) and be an active state Nurse Practitioners license holder. They should also possess current Nurse Practitioner National Board Certification, active state DEA licensure, and five or more years of nursing experience, preferably with at least two years of nurse practitioner experience.
The successful candidate will be responsible for performing evidence-based medical care, examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses. They will also order and interpret laboratory and diagnostic testing as needed, collaborate with caregivers and other healthcare professionals, provide referrals to other healthcare practitioners when necessary, and ensure patient medical records are kept up to date.
