Job Summary

Summit Home Care and Hospice is seeking a skilled Physical Therapist Assistant to join our team of healthcare professionals. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities
  • Administer various physical modalities, such as soft tissue massage, electrical muscle stimulation, and ultrasounds, to help relieve pain, improve circulation, and decrease swelling in the muscles.
  • Collaborate with other healthcare professionals to achieve well-rounded patient care.
  • Develop and implement physical therapy programs that focus on rehabilitation and patient recovery.
  • Evaluate the effects of therapy treatment and communicate patient progress to the healthcare team.
  • Observe and reassess patient progress to modify the plan of care as needed.
  • Provide personalized care to patients and ensure they receive the best possible treatment.
  • Train patients on the use of assistive devices, such as crutches, canes, or prosthetics.
  • Record and document patient care services to track progress and identify areas for improvement.
Requirements
  • Graduate of an accredited physical therapy assistant program with an Associate's degree in science or applied science approved by the American Physical Therapy Association.
  • Current certification or license as required by state law.
  • Demonstrates good verbal and written communication and organization skills.
  • Possess and maintain current CPR certification.
  • Licensed driver with a vehicle in good working order and insured in accordance with the organization's requirements.
